I am aware of the current Australian Government regulations to wear a mask within airports and on board the aircraft and am willing to follow this directive.

If you are suffering from any illness, disease or other conditions which may make it unsafe for you or other passengers if you fly, you must inform Alliance prior to your travel.
If Alliance, acting reasonably, considers that it may not be safe for you, or other passengers if you fly, Alliance may refuse to carry you.
